A 911 dispatch system routed a woman's call 200 miles away. She was having a heart attack. The ambulance arrived 45 minutes late.

The system's routing rule was simple: IF the caller's registered address is in Zone A, THEN route to Dispatcher A. She called from her cellphone. Cellphones don't have registered addresses. The condition returned false for every zone. Her call went to a default queue -- 200 miles from where she was standing.
